Enrollment

To enroll your child in Alpine Elementary School please print and fill out all applicable forms and submit to the school office.

Please include the following documentation.

Birth certificate or other legal proof of place of birth
Copy of immunization record
Proof of residency and required documents (listed on form)
Copy of custody documentation
